Registration and Intake paperwork must be completed by your first counseling session.
By filling out these forms ahead of time, it gives us more time to talk about why you are coming in, and less time filling out the paperwork. You can complete this paperwork on your computer, cell phone, or tablet. Please let your Counselor know if you are not able to complete this paperwork on any of those electronic devices.
If you’re a new client, please click on the link below. You will be redirected to a secure page to fill out your paperwork. You are going to look for your Counselor’s name and click on the link to the right (Adult or Child/Adolescent). This paperwork will take you approximately 20-25 minutes to complete. If you cannot finish, please click the “Save and Continue Later” so you do not need to start completely over again when you have time. If you are only needing to complete one or two pages (your provider will notify you which paperwork is needed), please follow the same link and go to the bottom of the screen. You will find individual pages of paperwork instead of the entire intake paperwork packet. Each individual that is participating in sessions needs to fill out a registration packet. If you would like us to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), complete this form to consent release of psychotherapy information.
If you would like us to share with someone other than a provider (for example, your teacher, family member, friend, attorneys, Department of Human Services, employer, etc.), complete this form to authorize release of psychotherapy information. This document is also included in the “New Client” paperwork packets.
